Fragrance Free Office Policy
Please Do Not enter Dr. Dore’s office if you or anyone with you is wearing any perfume/cologne or any other scented products to an appointment. Your appointment will need to be rescheduled if you are wearing any scented products.
Insurance
Dr. Dore is a Medicare Provider.
Dr Dore is a Blue Shield of California PPO Provider (Commercial/Group Plans Only).
Dr Dore is considered “Out of Network” with all Blue Shield of CA “Individual/Family Plans” (IFP), eg, Platinum, Gold, Silver and Bronze Plans.
It is the patient’s responsibility to call and confirm with their own insurance plan to confirm.
The receptionist will ask to verify your insurance information, address, and telephone number at the time you check in for your appointment. This is necessary in order to correctly process your insurance claim.
Dr. Dore does not provide consultations for Workers’ Compensation, accident and/or lawsuit cases.
Billing
Our office will bill Blue Shield of California and/or Medicare. Patients are responsible for payment of your deductible, co-pay, and any non-covered services. New Patient Referrals
Dr. Dore is a specialist; most insurance companies require a written referral requested by your primary care physician, internist or other doctor. Even if your insurance company does not require a written referral, Dr. Dore requires a written referral.
Dr. Dore does not see Self-referrals or non-physician referred patients.
If you do not have the required referral, you will have to reschedule your appointment until the referral is received by our office. These are policies required by your insurance companies and /or Dr. Dore’s office policies.
All patients must have a primary physician or other health-care provider for general medical (and emergency) care as well as routine physical exams. For example, your primary physician is responsible for your routine screening for various diseases, such as for breast, cervical, colon and prostate cancer. Breast, genitourinary and rectal examinations are not performed by Dr. Dore as part of her rheumatology care.
